Dust off the ol' textbooks!

I thought it might be interesting to provide a diagnostic challenge, just one of the many conundrums I've faced here.
History: Middle-aged man complains of recurrent sores and lumps all over his body for over a year now, most painful/bothersome on his hands as these crack, peel and bleed from farming and chopping with hand tools. Also reports discolored spots to palms and soles of his feet for a while now.
Pertinent findings on exam: Macular, irregular, hyper-pigmented lesions to palms and soles with numerous nodular sores throughout face, trunk and extremities. No cervical or axillary lymphadenopathy.
